Texas Health & Safety Code §181.105

Your Right to Access Health Care Records

As a client of Revelation Counseling Services, you have the right to request access to your health care records at any time. This right may be exercised by the client directly or by a legally authorized representative acting on the client’s behalf.

How to Submit a Records Request

To request your records, please submit a written request that includes all of the following:

  1. Your full legal name

  2. Date of birth

  3. A description of the specific records being requested

  4. Your preferred method of delivery (e.g., email, mail, in-person pickup)

  5. Your signature and the date of the request
